Register by Mail

Please note: CCC&TI is a PCI compliant organization. As such, we only accept credit cards (MasterCard and Visa) as payment via Self Service (online registration) and when cardholder registers in person . We do NOT accept credit card via fax or US Mail.

To register by mail:

1. If you don’t have it, download & install Adobe Reader.
2. Download our Registration Form
3. Use your computer to fill out the registration form
4. Print the form
5. Mail the form and a check or money order for your payment to:
